Staffing and scheduling: The HCWs shall provide services Monday through Friday between the hours of 7:30am and 4:00pm. Qualifications: The healthcare workers (HCWs) shall possess and maintain the minimum qualifications stated in the following: Possess education as follows: Either a Bachelor’s Degree in Nursing; or a Diploma Graduate in Nursing; or an Associate Degree in Nursing from an accredited university. Possess a current unrestricted license to practice as a registered nurse in one of the fifty states, the District of Columbia, the Commonwealth of Puerto Rico, Guam, or the U.S. Virgin Islands. Possess current certification in a minimum of one of the following: (1) Certified Case Manager (CCM) issued by the Commission for Case Manager Certification (2) Certified Disability Management Specialist (CDMS) issued by the Certification of Disability Management Specialists Commission (3) Certified Rehabilitation Registered Nurse (CRRN) issued by the Association of Rehabilitation Nurses (4) Certified Occupational Health Nurse (COHN) issued by the American Board for Occupational Health Nurses (5) Advanced Certification in Continuity of Care (ACCC) issued by the National Board for Certification in Continuity of Care (6) Certified Rehabilitation Counselor (CRC) issued by the Commission on Rehabilitation Counselor Certification (7) Nurse Case Manager (RN-NCM) issued by the American Nurses Credentialing Center (8) Care Manager Certified (CMC) issued by the National Academy of Certified Care Managers - OR – Possess a minimum of 2 years full-time experience within the last 3 years as a registered nurse providing case management and obtain one of the certifications above within 24 months of commencing service under this contract - OR - Possess a Masters degree in Nurse Case Management and obtain one of the certifications above within 24 months of commencing service under this contract. Possess a valid driver’s license and a personal vehicle for use under the contract. Possess the necessary knowledge, skills and computer literacy to interpret and apply medical care criteria such as InterQual. Possess a Master’s Degree in Nursing from an accredited university. Possess a minimum of 3 consecutive full-time years of employment within the last 5 years as a registered nurse providing case management with one of the certifications specified above.
